1) Completed and signed member service agreement
2) Copy of business license
3) Business checking statement or cancelled check

Landlords — (return the following items via fax or mail)
1) Completed and signed landlord application
2) Landlord's personal identification which may included: copy of driver's license, state I.D., or passport
3) Proof of ownership of the rental property which may include: copy of property deed/ title, property tax return, property insurance document, or utility bill
4) Signed tenant application with tenant authorization
